We also help people with diabetes take charge of their own health and well-being, offering individual and group education programs to give patients the tools they need to avoid diabetes complications and maintain better health throughout their lives.
Our goals are to provide participants with diabetes self-management skills and knowledge to achieve and maintain blood glucose control, to initiate lifestyle changes, and to promote the continuation of wellness.
