Discharge

Your attending physician will decide when you may be discharged. We strive to have all patients discharged by 11:00 AM, but you may be discharged later, depending on your doctor's orders and any paperwork or procedures requiring completion.

You can help make the discharge process run smoothly. The day before discharge, please arrange a ride home for the following day. If you don't have a ride, tell your nurse and we will help arrange transportation.


Care Coordination

Care coordinators can help arrange care after your discharge from the hospital. This may include a rehabilitation or extended-care facility, home care or other services.

Your care coordinator can give you a list of area facilities and services that provide short-term rehabilitation, skilled nursing facilities, home care and others. Once you choose a service or services, your care coordinator will help you finalize arrangements. Your care coordinator can also help you identify what your insurer will cover, or direct you to financial or community resources that can help.
